When this consent is needed
The consent is executed as a separate document and is not part of the contract, application or other documents (Part 1 of Article 9 of Federal Law No. 152-FZ of 27.07.2006 "On Personal Data").
Consent is not required for concluding and performing a contract for communication services, for settlements and for the performance of the duties of a communications operator: the Operator processes data on the basis of the contract and the law (clauses 2 and 5 of Part 1 of Article 6 of Law No. 152-FZ, Articles 53 and 64 of the Federal Law "On Communications"). Refusal to give consent under this form does not affect the conclusion of the contract or the provision of services.
Consent is needed only for the purposes marked in section 3 and for sending a message through the form on the website (section 7). The processing procedure is described in detail in the Personal Data Processing Policy.

The Operator does not entrust the processing of the data listed below to third parties. If such a person appears, its name and address will be stated in a new edition of the form, and new consent will be requested before the data is transferred.
3. Purposes, data and periods
I give consent separately for each purpose marked (tick the ones that apply):

4. Actions with data and processing methods
Collection, recording, systematization, accumulation, storage, clarification (updating, modification), retrieval, use, blocking, deletion, destruction. Processing is carried out both with and without the use of automation tools. The data is not transferred (disseminated, provided) to third parties, and no cross-border transfer takes place. The data is stored on the Operator's servers in the Russian Federation.
5. Withdrawal of consent
Consent for any of the purposes may be withdrawn at any time: by a letter to the Operator's address, by email to abonent (at) bravotele.com or through the Customer Account. Advertising messages can also be declined by replying to a message or by informing the Operator by phone at 8-800-551-00-95.
After withdrawal, the Operator stops processing for that purpose and destroys the data within 30 days, unless there is another lawful basis for processing it (Part 5 of Article 21 of Law No. 152-FZ). Advertising messages are stopped immediately.
6. Confirmation
I confirm that I give consent of my own free will and in my own interest, and that the consent is specific, substantive, informed, conscious and unambiguous. I have read the Personal Data Processing Policy and my rights (Article 14 of Law No. 152-FZ).

7. Consent when sending the "Write to us" form on the website
By ticking the consent box in the form on the bravotele.com website and sending a message, I give consent to "Scan-City" LLC (INN 2721167151, OGRN 1092721002615, 30 Zaparina St., apt. 60, Khabarovsk, 680000) to the processing of my personal data on the following terms:

Consent is given separately from other documents, by a mark in the form that is not ticked by default. A message cannot be sent without consent; the Operator can also be contacted without the form - by phone at 8-800-551-00-95 or by email.
